To begin your participation in Ohio Green Fleets, you'll need to fill out the Ohio Green Fleets Enrollment Form (a PDF that can be viewed using the free Adobe Acrobat Reader). This form is easy to fill out and gives us the information we need to begin the enrollment process. We will be in touch with you shortly after we receive this form to continue the process and determine if we require more information.

Next, you'll need to download and fill out the Ohio Green Fleets Data Submission Form, which is a downloadable Excel spreadsheet. Please note that there are potentially three tabs you will need to complete. This spreadsheet gives us the information to be able to evaluate your fleet and determine which level of Green Fleet certification your fleet has attained.
